The CFC program, or Community First Choice, is a State Medicaid-funded program that aids seniors and physically disabled individuals.
Care services are provided at home, the home of a loved one, or in a foster care home.

The program provides assistance with activities of daily living to Medicaid recipients who have a chronic illness, medical condition, or disability. Services are provided in the eligible individual’s home or community residence (waiver participants may receive services in an assisted living facility). Other services in each program vary.

How Do I Apply To Receive These Services?

To apply, please contact the Maryland Department of Health at 410-767-1739. Qualified applicants must meet both the Medicaid Program’s financial and medical requirements. An assessment completed by your Local Health Department determines medical eligibility. Financial eligibility is determined by either the local Department of Social Services or by the Department of Health and Mental Hygiene.

• Transition Services
• Home Delivered Meals

CFC Participants are also eligible to receive other Medicaid services which may include:

• Physician and Hospital Care
• Pharmacy
• Home Health
• Laboratory Services
• Mental Health Services
• Disposable Medical Supplies
• Durable Medical Equipment

WHO SHOULD APPLY
Maryland residents who need support with activities of daily living, such as bathing, grooming, dressing, and getting around.
ELIGIBILITY GUIDELINES:

Medical and Technical Criteria

  • Individuals must reside in the community.
  • Individuals must meet the level of care required to qualify for services in an institution.

 

Financial Criteria

  • Individuals can apply for Medicaid at anytime
  • Individuals who receive Supplemental Security Income (SSI) are automatically eligible for Medicaid and do not need to apply
